# Risk Register

A risk is a future thing that might go wrong; an issue is a risk that did. Tracking risks lets you act before they crystallize, when mitigation is cheap.

## Fields per risk
- **Description.** What might happen and why.
- **Likelihood × Impact.** Rank high/high first.
- **Owner.** Someone specific, not "the team".
- **Mitigation.** What you're doing to reduce likelihood or impact.
- **Trigger.** The observable signal that it's becoming real, so you act at the right moment.

## Use
- Review weekly. A static register is theater.
- Risks convert to issues; close them when mitigated or no longer relevant. A register that never shrinks isn't being used.
